202507183 <br />Exhibit "A" 12/10/2025 <br />Solid, pink line is a proposed <br />route for new 2" Sch40 <br />conduit. <br />• Place cabinet on west side of existing structures. Cabinet dimensions:52"hx28"dx30"w. There is existing <br />2" Sch40 in existing low pedestal that will be used to go east then south. <br />• Existing 2" Sch40 stubbed up. This will be coupled to new 2" Sch40 and buried. <br />• Handhold/Vault. 24"x36"X24" placed flush with grade. <br />• Conduit will enter existing facilities. <br />The Grand Island Streets Division shall be contacted prior to work beginning at 308-385-5322. <br />A Notice to Proceed with work in the ROW will not be given until a pre -construction notice has been given; the City of Grand Island permit <br />number is required when requesting the Notice to Proceed. <br />Any keyhole potholing locations in the roadway shall be approved by the Streets Division and shall be filled within five days of potholing. <br />No handholds or vaults shall be placed in the flowline or sloped bank of drainage ditches. If it has to be placed on sloped bank the final grade of <br />hand hold or vault lid shall match that of the sloped bank. <br />Erosion control shall be in place for all open excavations adjacent to drainage ditches. All drainage ditch flowlines must be restored before job is <br />done. <br />No handholds, vaults, or manholes shall be placed in the ADA accessible area of the sidewalk. <br />Any handholds, vaults or manholes placed in the sidewalk shall match the existing slopes of the sidewalk and shall not create a tripping hazard <br />for pedestrians. <br />Any sidewalk removals & replacements shall be approved by the City of Grand Island prior to removal. Restoration of sidewalk shall be done <br />within 5 days of removal and shall be inspected by the City of Grand Island Public Works Department. <br />Any street or sidewalk closures require a 72-hour notice to the City of Grand Island Public Works Department at 308-385-5444. <br />Contractor will be required to request a locate before any physical work is to take place. Locate flags shall be removed after the "excavator" has <br />determined the work is complete. <br />All ROW shall be restored to the same condition as prior to the work taking place which includes but is not limited to placement of top soil & <br />seeding at excavated areas within green space and repairing washouts or ruts from construction, seeding disturbed earth, and seeding & <br />erosion control on sloped banks after disturbed. A ROW inspection shall be completed once work is done. Contact the Grand Island Streets <br />Division at 308-385-5322 to request an inspection. <br />Where CROSSING SIGNAL INFRASTRUCTURE <br />Conduit shall not be placed between the City's traffic signal cabinet and connecting traffic signal conduit/poles. Conduit must be offset a <br />minimum of 5' from the base of any traffic signal cabinet, traffic signal pole, or traffic signal pull box. <br />IF SIGNAL CONDUIT/WIRE HAS TO BE CROSSED THE NEW LINE SHALL BE A MINIMUM OF 6' DEEP WHERE CROSSING AND THE <br />EXSTING SIGNAL CONDUIT SHALL BE POTHOLED AND VERIFIED ON -SITE PRIOR TO BORING. <br />
