A Notice to Proceed with work <br />in the ROW will not be given until a pre -construction notice has <br />been given; the City of Grand Island permit number is required <br />when requesting the Notice to Proceed. Any keyhole potholing <br />locations in the roadway shall be approved by the Streets <br />Division and shall be filled within five days of potholing. No <br />handholds or vaults shall be placed in the flowline or sloped bank <br />of drainage ditches. If it has to be placed on sloped bank the <br />final grade of hand hold or vault lid shall match that of the <br />sloped bank. Erosion control shall be in place for all open <br />excavations adjacent to drainage ditches. All drainage ditch <br />flowlines must be restored before job is done. No handholds, <br />vaults, or manholes shall be placed in the ADA accessible area of <br />the sidewalk. Any handholds, vaults or manholes placed in the <br />sidewalk shall match the existing slopes of the sidewalk and shall <br />not create a tripping hazard for pedestrians. Any sidewalk <br />& replacements shall be approved by the City of Grand <br />Island prior to removal. Restoration of sidewalk shall be done <br />within 5 days of removal and shall be inspected by the City of <br />Grand Island Public Works Department. Any street or sidewalk <br />closures require a 72-hour notice to the City of Grand Island <br />Public Works Department at 308-385-5444. Conduit must be offset a <br />minimum of 5' from the base of any traffic signal cabinet, traffic <br />signal pole, or traffic signal pull box.
