Wood River, NE 68883 (Address), with the <br />license number of issued on (mm/dd/yyyy) <br />and expiring on (mm/dd/yyyy), (the "Claimant"), claims a construction lien <br />in the sum of $6532.00 for labor, services, materials, and/or equipment furnished for <br />improvement to certain real property owned by Wade and Courtney Rose (collectively, the <br />"Owner"), located at 605 Oasis Pl. Carlo, Ne 68824 (Address w/County), <br />and with the legal description of: <br />Lot (4) and (5), Weber Subdivision, City of <br />(Cario, Hall county, Nebraska <br />(the "Property"). <br />This Lien is claimed, separately and severally, as to both the buildings and improvements <br />thereon, and the said real property. <br />The Claimant and Wade/Courtney Rose (Owner, General Contractor, etc.) entered into a <br />contract on 05/01/2023 (mm/dd/yyyy) whereby the Claimant provided the following <br />labor, services, materials, and/or equipment at the Property (the 'Work"): <br />Installation of new windows, doors, and siding <br />for the total amount of $21,500 (Contract Price). <br />eSign <br />Page 1 of 3 <br />
