PROJECT NOTES <br />T. THE LOCATKk1S OF ALL AERIAL AND UNDERGROUND UTILITY FACLITES <br />MAY NOT BE INDICATED ON THESE PLANS. UNDERGROUND UTILITIES, <br />WHETHER SHOWN OR NOT, WILL BE LOCATED AND FLAGGED BY THE <br />UTLJTIES UPON THE REQUEST OF THE CONTRACTOR. NO EXCAVATION <br />WILL BE PERMITTED N THE AREA OF THE UNDERGROUND UTILITES <br />UNTIL ALL SUCH FACLITIES HAVE BEEN LOCATED AND IDENTIFIED TO <br />THE SATISFACTION OF ALL PARTIES. THE CONTRACTOR SHALL BE <br />RESPONSIBLE FOR PROTECTION OF ALL UNDERGROUND AND AERIAL <br />UTILITIES AND CONSTRUCTIONS. <br />2. UNLESS OTHERWISE AGREED TO IN WRITING IN ADVANCE OF THE <br />PROJECT CONSTRUCTION, THE DEPTH OF INSTALLED FACLITIES <br />SHALL BE, AT A MINIMUM, AS FOLLOWS: <br />A. FORTY-TWO (42) INCHES IN SOL. <br />B. FORTY-TWO (42) INCHES BELOW A PROJECTED SLOPE FROM THE <br />FLOW LINE OF A DRCH AT A THREE (3) HORIZONTAL AND ONE (1) <br />VERTICAL SLOPE. <br />C. FORTY-EIGHT (48) INCHES UNDER A ROADWAY MEASURED FROM <br />THE SURFACE OF SAID ROADWAY TO THE TOP OF THE <br />INSTALLATION. <br />D. FORTYEIGHT (48) INCHES UNDER A STORM WATER OR CREEK <br />CHANNEL DESIGN FLOW LINE, AND <br />E. MAINTAIN A MINIMUM OF 24.OF VERTICAL & HORIZONTAL <br />SPACING FROM EXISTING UTILITIES. <br />3. ALL POTHOLES IN SIDEWALK PANELS WILL REQUIRE FURL PANEL <br />REPLACEMENT. <br />4. ANY CUT OR POTHOLE IN A CONCRETE STREET PANEL WILL REQURE <br />FULL PANEL REPLACEMENT OR CORE REPLACEMENT WITH AN <br />APPROVED EPDXY. <br />5. ANY CUT OR POTHOLE IN AN ASPHALT STREET LANE WILL REQUIRE SEAM <br />TO SEAM ASPHALT MILLING AND REPLACEMENT. <br />6. ALL WORK ZONE TRAFFIC CONTROL, INCLUDING PEDESTRIAN CONTROL <br />MEASURES. SHALL BE IN COMPLIANCE WITH THE MUTCD, LATEST EDITION, <br />AND ADA. <br />7. PULL STRING SHALL BE INSTALLED M EACH AND EVERY NEW CONDUIT. <br />CITY OF GRAND ISLAND NOTES <br />1. CONDLRT <br />MUST BE OFFSET A MINIMUM OF 5' FROM THE BASE OF ANY TRAFFIC SIGNAL <br />CABINET, TRAFFIC SIGNAL POLE, OR TRAFFIC SIGNAL PULL BOX. <br />2. CONTRACTOR WILL BE REQUIRED TO OSTAN A PERMIT TO WORK IN THE <br />ROW (INCUJ0IN0 A TRAFFIC CONTROL PLAN) BEFORE ANY PHYSICAL WORK <br />IS TO TAKE PLACE, <br />3.
