Exhibit "A" 07/08/2024 <br />CITY OF GRAND ISLAND <br />HALL COUNTY <br />The Grand Island Streets Division shall be <br />contacted prior to work beginning at <br />308-385-5322. <br />A Notice to Proceed with work in the ROW will <br />not be given until a pre -construction notice has <br />been done; the City of Grand Island permit <br />number is required when requesting the Notice <br />to Proceed. <br />Any keyhole potholing locations in the roadway <br />shall be approved by the Streets Division and <br />shall be filled within five days of potholing. <br />No handholds or vaults shall be placed in the <br />flowline or sloped bank of drainage ditches. If It <br />has to be placed on sloped bank the final grade <br />of hand hold or vault lid shall match that of the <br />sloped bank. <br />Erosion control shall be in place for all open <br />excavations adjacent to drainage ditches. All <br />drainage ditch flowlines must be restored before <br />job is done. <br />19' IN EASEMENT <br />No handholds, vaults, or manholes shall be <br />placed in the ADA accessible area of the <br />sidewalk. <br />Any handholds, vaults or manholes placed in the <br />sidewalk shall match the existing slopes of the <br />sidewalk and shall not be or create a tripping <br />hazard for pedestrians. <br />Any sidewalk removals & replacements shall be <br />approved by the City of Grand Island prior to <br />removal. Restoration of sidewalk shall be done <br />within 5 days of removal and shall be inspected <br />by the City of Grand Island Public Works <br />Department. <br />Any street or sidewalk closures require a 72 - <br />hour notice to the City of Grand Island Public <br />Works Department at 308-385-5444. <br />"Maintain 3' of horizontal separation <br />if paralleling water mains. Maintain <br />1' of vertical separation at water <br />main crossings." <br />00. <br />Contractor will be required to request a locate <br />before any physical work is to take place. Locate <br />flags shall be removed after the "excavator" has <br />determined the work is complete. <br />A ROW inspection will be completed once work <br />is done.
