.r . : . 9g' 1C39 , 1� • DURABLE POWER O F ATTORNEY KNOW ALL MEN BY THESE PRESENCE, that I, FLORENCE MAE SLOCUM (Social Security No. 256-30-4710), whose address is 611 West 12th Street, Grand Island, Nebraska; and first being duly swom upon oath, does hereby make, constitute, and appoint my daughter, KATHLEEN L. JEWETT whose address is 319 East 20th Street, Grand Island, Nebraska, my true and lawful attorney in fact for me and in my name, place and stead, and on my behalf and for my use and benefit: 1. To do all business dealings such as signing checks, cashing certificates of deposit or other types of money making certificates, to sign, seal, execute and deliver and acknowledge such deeds, leases, and assignments of leases, covenants, indentures, agreements, mortgages, notes, receipts, and to all things necessary in the handling of my private affairs. 2. To sell or exchange real or personal estate and be able to sell either by public or private sale any part of my real estate or personal property for such consideration upon such terms as my attomey shall think fit, and to execute and deliver good and sufficient deeds, bills of sale, endorsements and assignments. 3. To execute and sign motor vehicle titles and title to trailers and to sign all things necessary for the selling and transfer of said tides. 4. Pursuant to Nebraska Revised Statutes Sections 30-2662 and 30-2663, this Power of Attorney shall rema,in in effect during any disability or incapacity that I might have as a result of physical or mental conditions. All such authority contained in this Power of Attorney shall continue after my death, until notice of my death has actually received by my attorney. Any action taken by my attorney while I am disabled or incapacitated or before actual notice of my death has been received, shall be valid as if I were alive, competent and not disabled. To sign any and all papers relating to my present or future medical problems and to consent to surgery and hospitalization, to do any and all things necessary in making decisions in my behalf. 5. As a part of this Power of Attorney the undersigned FLORENCE MAE SLOCUM does hereby make direction to be followed in the case of serious sickness. I trust and have faith in my designee, KATHLEEN L. JEWETT, and designate her to make the decision as to whether life support systems should be maintained on my body simply to prolong my certain death. If I am brain dead or have such other incurable and irretrievable body conditions that result in no recovery expectations, I direct that my physician withhold or withdraw the DURABLE POWER OF ATTORNEY - Page 1 of 2
