Worth Ct. <br />Grand Island NE 68803 <br />Notice to Register of Deeds <br />This is a transfer on death deed and Form 521 is not required with the filing of this Transfer on <br />Death deed under Neb. Rev. Stat. § 76-214(2)(b). <br />Legal Description <br />The following real estate is transferred by this document, effective at the death of the Owner to <br />the Primary Beneficiary or to the Alternate Beneficiary as provided below: <br />Lot Eight (8) in Capital Heights Fifth Subdivision, being a part of the Southwest Quarter <br />(SW1/4) of Section Two (2), Township Eleven (11) North, Range Ten (10) West of the 6th P.M., <br />in Hall County, Nebraska. <br />Primary Beneficiary <br />I designate the following beneficiaries in equal shares as tenants in common, of the real estate <br />covered by this revocable transfer of death deed if the beneficiary survives me: <br />Judith Ann Gowlevech, 4201 Texas Ave, Grand Island NE 68803. <br />Edward J. Gowlevech, 35402 243 Rd Street, Atchison KS 66002. <br />Colleen Ann Guerra, 329 Frank Baum Dr., Schertz TX 78154. <br />Elizabeth Ann Porath, 617 W Ruhl Rd, Palatine IL 60074. <br />Alternate Beneficiary <br />If any named beneficiary predeceases me, their share shall be divided among the surviving <br />primary beneficiaries. <br />Transfer on Death <br />At my death, I transfer my interest in the described property to the beneficiaries as designated <br />above.
