STAKING SHEET- UNDERGROUND <br />Company: OPTK Networks Exchange: Beatrice <br />CONSTRUCTION PROJECT NOTES <br />1 The State, County. City personnel 5h311 be notified a minimum of forty eight i 481 hours in advance of Initial project <br />construction <br />2. The utility company and the oontractortsi are responsible to have at least one person on the lob at an times. that is <br />'site supervisor certified. <br />3, The contractor shall adhere to the current permit and contract specifications_ <br />4. The locations of all aerial arid underground utility facilities may not be rid on the project plans The contractor <br />is required to contact the Nebraska and Colorado 811 one call notification center (dial 8111 forty-eight 4131 hours <br />poor to working in the city right of way. No excavation will be permitted in the area of the underground utilities until <br />all facilities have been located and identified to the satisfaction of all parties. The contractor shall be responsible for <br />protection of all underground and anal utilities and infrastructure. <br />5 Whenever underground facilities are in close proximity to the proposed pathway location or whenever the vertical <br />location of the underground facility is unknown. the contractor shall use special measures to determine the locations <br />of such underground facilities. <br />a If the contractor is conducting the excavation by digging from the surface when on a close pro• unity to the <br />underground facilities. the contractor shall use hand digging, hydro ercavatng, or air excavating to locate such <br />facikty. <br />b N the contractor is conducting the excavation by tunnekng or boring. the contractor shall determine the vertical <br />location of the underground facility by pothokrg or any other method approved by the director of public works and <br />utilities <br />el Project plans shall dimension new facilities being installed <br />7. Unless agreed to in venting to advance the depth of instated facilities in city and county row shall be at a minimum <br />as follows <br />a. Rorty -eight (48) inches in soil <br />b. Rorty -eight (48) inches below a projected slope 'Torn the flow lone of a ditch at a three (31 horizontal and one I 1 I <br />vertical slope. <br />c. Rorty -eight (48) inches under a roadway measured from the surface of said roadway to the top of the mstalla�on <br />d. Maintain a minimum of twenty-four (24 1 Inches of vertical and horizontal spacing from eristng utilities <br />f Be boated as far from the existing or proposed curb one as possible to avoid potential future conflicts <br />B. All non -street surface materials that are disturbed by eccavabon and backfilling operations shin be replaced and <br />restored <br />8. City and county street surface materials disturbed shall be backfilled. tamped and repaved by city or county forces <br />or approved paving contractor <br />10. If sidewalk. wheel chair ramps or any portion of the pedestrian facilites are removed or dosed, a pedestrian detour <br />shall be established and maintained during the time of the closure and the entire pedestrian facility shall be restored <br />within seventy-two (72) hours. unless the contractor provides a hard surface alternate route approved by the city or <br />county <br />11 All pedestrian detours and any reconstruction of pedestrian facilities shall meet Current Americans with Disability <br />Act (ADA) standards and specifications <br />12 All pedestrian detours shall be part of an approved Traffic Control Plan (TCP I <br />3. All holes made In the streets for the purpose of performing vacuum excavations to locate underground utilities shall <br />be restored to their onginal condition with the reinstated core flush with and in the original orientation as the costing <br />surface matching existing pavement surface appearance. <br />t4. When the contractor is not actively working, all equipment, fencing, debris, etc. Shall not be placed within the <br />triangular area required for sigh! distance of vehicles exiting or entenng an adjacent property or intersection. <br />15 All excavations shat be adequately fenced and covered when contractor is not present or proles site left <br />unattended. <br />18. No lane closures allowed on arterial streets dung AM and PM peak hours. <br />17. All work zone traffic control including pedestrian control measures. shall be in compliance with the MUTCD and <br />ADA <br />18. The contractor stall mamtan a set of "as built' plans on site with dimensioning. These plans shall be presented <br />upon request, <br />it Final "as -built" plans shall be Riad with city and/or county within 30 days of the completion of the project. <br />20. The contractor shall follow construction standards pertaining to the lateral corridor facility placement and minimum <br />depth reouirernents and clearance requirements of other facilities <br />2'. A current State, County or City permit shall be maintained on each work site by the contractor. The contractor <br />shall present such permit upon request <br />22. Any field adjustments to installation of facilities, which vary from the plans that have beer submitted and approved <br />during the permit application process. shall ooniply with the following <br />a The contractor must stop work immediately and contact the facility owner of the proposed changes <br />b The facility owners representative shall contact the city, county or state right-of-way construction inspector and <br />provide them with the details. <br />c. The ROW inspector will make a determination on tow to proceed. This determination may range from approval <br />over the phone or via email to delaying the project until plans can be updated and appropriate review can be <br />conducted <br />23. Stake mounted waning signs and marker posts (BM531 to be placed where appropriate <br />24. Alt conduit and fiber is to be installed at the back of the roadway ROW unless otherwise noted <br />25.
