Under date of July 2, 1991, Lucille Delaney created the Lucille Delaney Trust <br />which she amended on February 3, 1998, April 27, 2004, June 29, 2007, March 31, 2010, <br />November 29, 2011, March 25, 2013, March 5, 2014, July 25, 2017 and September 21, 2017 (the <br />"Lucille Delaney Trust") wherein she designated herself as the initial Trustee of the Trust; <br />2. On July 25, 2017, Lucille Delaney withdrew as Trustee of said Lucille Delaney <br />Trust and designated Loren Haselhorst as the Trustee of the Trust; <br />3. From July 25, 2017 through the date hereof, Loren Haselhorst has continued to <br />serve as Trustee of said Lucille Delaney Trust; <br />4. • Lucille Delaney died on July 16, 2022 and as a result of the death of Lucille <br />Delaney, the Lucille Delaney Trust became irrevocable; <br />5. The State of Nebraska is the jurisdiction under which the Lucille Delaney Trust <br />was established; <br />6. Affiant, Loren Haselhorst is currently serving as Trustee of the Lucille Delaney <br />Trust and said Trust grants Affiant as Trustee all powers under the Nebraska Uniform Trust <br />Code; <br />7. The Lucille Delaney Trust is in full force and effect as of the date hereof; <br />8. The Internal Revenue Service has issued a taxpayer identification number for the <br />Lucille Delaney Trust; <br />9. Julie A. Cahoj, Kristine D. Hanson, Lynda L. Delaney and Terri L. Haselhorst are <br />the beneficiaries of the Lucille Delaney Trust; and <br />1 <br />
