202301743 <br />7. If the Father desires to have in-person parenting time with the <br />minor children, the Father shall contact the Mother to schedule therapeutic sessions <br />to first occur between himself and the minor children. Visits shall begin <br />therapeutically and shall remain therapeutic unless and until the therapist for the <br />minor children determines that it is in the children's best interests to transition to <br />visitation outside of the therapeutic setting. <br />8. SCHOOL RECORDS AND MEDICAL TREATMENT: The <br />names of both parents shall appear on all school records. The names of both <br />parents shall appear on all medical records and both shall have access to all such <br />information. Each parent is responsible for notifying the school that s/he wishes to <br />be included on the mailing list and be notified of conferences and events, as well <br />as receiving copies of report cards, progress reports and all other pertinent <br />information. Both parents may initiate emergency medical and dental services for <br />the children and shall notify the other of any significant illness, injury, or <br />emergency medical treatment of the children. Both parents shall be aware of the <br />names of the current treating physician or dentist for the children at all times. <br />9. DECISION MAKING AND PARENTAL <br />COMMUNICATION: That due to past incidents of domestic violence between <br />the Father and the Mother as well as the Father's limited involvement with the <br />children, there shall be no need for the parties to discuss matters concerning the <br />children. The Mother shall communicate with the Father in the event of a <br />significant illness or death of the minor children. <br />10. CHANGE OF PARENT'S RESIDENCE: In the event that one <br />of the parents plans to change his/her residence, that parent shall notify the other <br />parent of such change of residence. If one of the parents is living or moving to an <br />undisclosed location because of safety concerns, the address or return address <br />shall only include the county and state. <br />11. CHANGE OF CHILDREN'S RESIDENCE: Both parents shall <br />provide notification to the other if s/he plans to change the residence of the <br />children. <br />12. TEMPORARY CHANGES: The parents can temporarily change <br />the terms of this Plan as long as they both agree to it. <br />Page /_ of ,0301 <br />2 <br />
