STAKING SHEET - UNDERGROUND <br />Company: OPTK Networks Exchange:Grand Island <br />CONSTRUCTION PROJECT NOTES <br />1. The State, County. City personnel shall be notifiea 3 minimum of forty eight (48) hours in advance of Initial project <br />construction <br />2 The utility company and the contactor(s) are responsible to have at least one person on the job at all times, that 5 <br />'see supervisor' certfied. <br />3. The contractor shall adhere to the current permit aha contract specifications <br />4. The locations of al i aenat arca underground utility facilities may not be Indicated on the proem plans. The contractor <br />is required to contact the Nebraska and Colorado 811 one call notification censer (dial 8t 1) forty-eight (48( hours <br />poor to working in the city right array. No excavation will be pemuUed in the area of the underground utilities until <br />ali facilites have been located and identified to the satisfaction of all parties. The aantractor shall be responsible for <br />protection of all underground and aerial utilities and nfrastructure. <br />Whenever underground faolties are on close proximity to the proposed pathway location or whenever the vertical <br />iocabon of the underground facility Is unknown, the contractor snail use special measures to determine the locations <br />of such underground facilites. <br />a the contractor is conductng the excavation by dogging from the surface wren in a close proximity to :he <br />underground fac4rtes the contractor snail use nand digging. hydro excavating. or ear excavating to locate such <br />facility. <br />b. t <br />the contractor is Cpn0udting the excavation by tunneling or boring. the contractor shall deterrnne the vertical <br />location of the underground faokty by pothokng or any other method approved by the director of public works and <br />utilrbes <br />t.
