CONSTRUCTION PROJECT NOTES <br />1. The State. County, City personnel shall be notified a minimum of forty eight (48) hours in advance of initial project <br />oonstn.cuon. <br />2. The utility company and the contractorts) are responsible to have at least one person on the job at all times. that .s <br />'site supervisor certified. <br />3. The contractor shaft adhere to the current permit and contract specifications. <br />4 The locations of all aerial and underground un8y facilities may not be indicated on the protect plans. The contractor <br />is required to contact the Nebraska and Colorado 811 one call notification center (alai 81 1) forty-eight (48) hours <br />prior to working in the city nght of way. No excavation will be permitted in the area of the underground unities until <br />all facilities have been located and dent(.ed to the sansfactio n of all parties. The contractor shall be responsible for <br />protection of all underground and aenal utilities and nfrasiructure. <br />5. Whenever underground facilities are in dose prox.mxty to the proposed pathway location or whenever the vertical <br />location of the underground facility .s unknown. the contractor shah use speaa( measures to deemrne the locations <br />of such underground facilities. <br />a if the contractor is conducting the excavation by digging from the surface. when in a dose proximity to the <br />underground facilities. the contractor stall use hand digging. hydro excavating. or air excavating to locate such <br />faciity. <br />b. if the contractor is conducing the excavation by tunneling or boring, the contractor snail determine the vertical <br />location of the underground facility by potholing or any other method approved by to director of public works and <br />ablaes <br />6. Project plans shall dimension new facilities being installed <br />7 Unless agreed to in wrong n advanoe the depth of installed facilities on city and county raw shall be at a minimum <br />as follows. <br />a Forty-eight (48) inches in soil <br />b. Forty-eight (48) inches below a projected slope from the flour fine of a ditch at a three (31 honzonta( and one (1) <br />vertical slope. <br />c Forty-eight (48) inches Levier a roadway measured from the surface of said roadway to the top at the installation. <br />d. Sony (d0) inches under a storm water or creek channel design totonn or pipe. and <br />e. Manan a minimum of twenty-four (24) inches of vertical and hormontai spacing from existing utilities <br />1. Be located as far from the existing or proposed cub lone as possible to avoid potential future conflicts <br />8. Al pedestrian detours and any reconstructor of pedestnan facilities shall meet current Americans with Disability <br />Act (ADA) standards and specifications. <br />12. AN pedestnan detours shah be part of an approved Traffic Comtrcl Plan (TCP). <br />13. AN holes shade in the streets for the purpose of perfommrrig vacuum excavations to locate underground utilities shall <br />be restored to them onginal condition with the reinstated core flush with and in to onginal onenation as the existing <br />surface matching eosting pavement surface appearance. <br />14. When the contractor is not actively working. all equipment. fencing. debris. etc. Shall not be paced within the <br />triangular area required for sight distance of ve1des exiling or entering an adjacent property or intersection. <br />15 AN excavations shall be adequately fenced and covered when contractor is not present or project site left <br />unattended. <br />16. No lane closures allowed on arsenal streets dung AM and PM peak hours. <br />17. All work zone traffic control, including pedestnan control measures, shall be in compliance with the MUTCD and <br />ADA. <br />18 The contractor shall maintain a set of "as built" plans on site with dimensioning. These plans shall be presented <br />upon request <br />18. Final "as -built" plans shall be filed with city andror county within 30 days of the completion of the project <br />20. The contractor shall follow construction standards pertaining to the lateral condor facility placement and minimum <br />depth requirements and clearance requirements of other facilities. <br />21. A current State. County, or Cry permit shall be maintained on each work sae by the contractor The contractor <br />shall present such permit upon request <br />22. Any field adjustments to installation ot faalrbes. which vary from the pans that have been submitted and approved <br />during the permit application process, shall comply with the following: <br />a. The contractor must stop work mmediately and contact the facility owner of the proposed changes. <br />b The facility owners representative shall contact the city, county or state right-of-way construction inspector and <br />provide them with the details. <br />c. The ROW inspector will make a detuvnuraoon on haw to proceed. This determonabon may range from approval <br />over the phone or via email to deayung the project until plans can be updated and appropriate review can be <br />conducted. <br />23. Stake mounted waning signs and marker posts (BA153) to be plaoed Where appropriate. <br />24. AN conduit and fiber is to be installed at the back of the roadway ROW unless othenrise noted. <br />25. Leave 120' loops (80' tails at spice points) for mainline fiber in vaults <br />Conduit shall not be placed between the City's traffic signal cabinet and connecting traffic signal <br />conduit/poles. <br />Conduit must be offset a minimum of 5' from the base of any traffic signal cabinet, traffic signal <br />pole, or traffic signal pull box. <br />Contractor will be required to obtain a permit to work in the ROW (including a traffic control plan) <br />before any physical work is to take place. <br />Locate flags shall be removed after the "excavator" has determined the work is complete. <br />IF SIGNAL CONDUIT/WIRE HAS TO BE CROSSED THE NEW LINE SHALL BE A MINIMUM <br />OF 6' DEEP WHERE CROSSING AND THE EXSTING SIGNAL CONDUIT SHALL BE <br />POTHOLED AND VERIFIED ON-SITE PRIOR TO BORING. <br />6T6f6JZ6v <br />
