STAKING SHEET- UNDERGROUND <br />Company: OPTK Networks Exchange: <br />CONSTRUCTION PROJECT NOTES <br />1. The State, County. City personnel shall be notified a minimum of forty eight (48) hours in advance of initial project <br />construction. <br />2. The utility company and the contractor(s) are responsible to have at least one person on the job at all tines, that is <br />'site supervisor' certified. <br />3. The contractor shall adhere to the current permit and contract specifications. <br />4. The locations of all aerial and underground utility facilities may not be indicated on the project plans. The contractor <br />is requited to contact the Nebraska and Colorado 811 one call notification center (dial 811) forty-eight (48) hours <br />prior to working in the city right of way. No excavation will be permitted in the area of the underground utilities until <br />all facilities have been located and identified to the satisfaction of ail parties. The contractor shall be responsible for <br />protection of all underground and aerial utilities and infrastructure. <br />5. Whenever underground facilities are in close proximity to the proposed pathway location or whenever the venial <br />location of the underground facility is unknown. the contractor shall use special measures to determine the locations <br />of such underground facilites. <br />a. If the contractor is conducting the excavation by digging horn the surface. when in a close pronmity to the <br />underground facilites. the contractor shall use hand digging. hydro excavating. or am -excavating to locate such <br />facility. <br />b. If the contractor a conducting the excavation by tunneling or boring. the contractor shall detente the vertical <br />location of the underground facility by potwtng or any other method approved by the director of public works and <br />utilities. <br />8. Project plans shall dimension new facikties being installed <br />7. Unless agreed to in writing n advance the depth of installed facilities in city and county row shall be at a minimum <br />as follows: <br />a. Forty-eight (48) inches in sod <br />b. Forty-eight (48) inches below a projected slope from the flow line of a ditch at a three (3) horizontal and one (1) <br />vertical slope. <br />c_ Forty-eight (48) inches under a roadway measured from the surface of said roadway to the top of the installation. <br />d. Sixty (00) inches under a storm water or creek channel design bottom or pipe. and <br />e. Maehtam a minimum of twenty-1our (24) aches of vertical and horizontal spall g from existing utilities_ <br />I. Be located as far fronh the existing or proposed cub line as possible to avoid potential future conflicts. <br />8. Al non -street surface materials that are disturbed by excavation and backfilling operations shall be replaced and <br />restored_ <br />0. City and county street surface materials disturbed shall be backfilled. tapped and repaved by oily or county forces <br />or approved paving contactor. <br />10. If sidewalk. wheel chair ramps or any portion of the pedestrian facilites are removed or dosed, a pedestrian detour <br />shall be established and maintained during the time of the closure and the enc re pedestrian facility shag be restored <br />within seventy -Mo (72) hours. unless the contractor provides a hard surface alternate route approved by to city or <br />11. Al pedestrian detours and any reconstruction of pedestnan facilities shall meet current Americans with Disability <br />Ad (ADA) standards and specifications. <br />12. Al pedestrian detours shat be part of an approved Traffic Control Plan (TCP). <br />13. Al holes made n to streets for the purpose of performing vacuum excavations to locate underground cites shall <br />be restored to their anginal condition with the reinstated core flush with and in to original orientation as the existing <br />surface matching existing pavement surface appearance. <br />14. When the contractor is not actively working: all equipment fencing. debris. ese. Shall not be placed within the <br />triangular area required for sght distance of vehicles exiting or entering an adjacent property or intersection. <br />15. Al excavations shall be adequately fenced and covered when eentrador is not present or project site left <br />unattended. <br />16. No lane closures allowed on arterial streets dump AM and PM peak hours. <br />17. All work zone relic control. inducing pedestrian control measures. shall be in compliance with the MUTCO and <br />ADA. <br />18. The contractor shall maintain a set of "as built' plans on site with dinensioreng. These plans shah be presented <br />upon request. <br />10. Final "as -bait plans sisal be filed with city and/or county withal 30 days of the completion of the project <br />20. The contractor shall follow construction standards pertaining to the lateral corridor facility placement and minimum <br />depth requirements and clearance requirements of other facilites. <br />21. A current State. Count. or City permit shall be maintained on each work site by the contractor. The contractor <br />shall present such permit upon request <br />22. Any held adjustments to mstallatcn of facilities, which vary from the plans that have been submitted and approved <br />during the permit application process. shall comply with the following: <br />a. The contactor must stop work immediately and contact the facility owner of the proposed changes. <br />b. The facility owner's representat ve shall contact the city. county or state right-of-way construction inspector and <br />provide therm with the details. <br />c. The ROW inspector will make a determination on how to proceed. This determination may range from approval <br />over to phone or via email to delaying to project until plans can be updated and appropriate review can be <br />conducted. <br />23. Stake mounted waning signs and marker posts (BM53) to be placed where appropriate. <br />24. All conduit and fiber is to be instated at the back of the roadway ROW unless otherwise noted. <br />25. Leave 120' loops (80' tails at spice points) for manline fiber in vaults <br />
