NONE <br />EFFECTIVE DATE <br />Power of attorney becomes effective upon disability of principal; continues in effect after <br />principal's death until notice. Pursuant to Section 30-4009 of the Nebraska Statutes, I declare <br />that this power of attorney shall become effective upon my disability or incapacity, but not until <br />then, and that the authority granted herein shall continue during any period while I am disabled or <br />incapacitated. I authorize my daughter, Heather Callihan, my daughter, Angela Ayres, or my <br />son, Andrew Case, to make such determination that I am disabled or incapacitated, if it is <br />necessary to establish my agent's authority to any person. Further, all such authority shall <br />continue after my death, until notice of such death shall have been received by my attorney so <br />that she has actual knowledge of the fact that I have died.
