Sapp Revocable Trust by Agreement dated May 24, 2001 which she subsequently amended and restated by that certain Amendment and Restatement of June M. Sapp Revocable Living Trust dated April 5, 2018 (the "Trust") wherein June M. Sapp designated herself as the initial Trustee of said Trust, and Maurice W. Sapp as the first successor Trustee. 2. From the date of the creation of the Trust until July 17, 2019, June M. Sapp served as Trustee of the Trust; 3. Upon the death of June M. Sapp and pursuant to ARTICLE XI of the Trust, Maurice W. Sapp executed a Declaration of Death of Trustee and Acceptance of Successor Trustee under date of Septemberg., 2019, wherein he has accepted the Trusteeship of said Trust and agreed to administer said Trust in accordance with the terms of the Trust Agreement and the laws of the State of Nebraska; 4. The Trust is irrevocable due to the death of the decedent, June M. Sapp and is in full force and effect; 5. Affiant Maurice W. Sapp is currently serving as Trustee of said Trust and the Trust Agreement grants Affiant as Trustee all powers under the Nebraska Uniform Trust Code; 6. Maurice W. Sapp is the beneficiary of the Trust; and 7. The Trust Agreement has not been revoked or amended to make any representa- tion contained in this Certification of Trust incorrect and the signature of Affiant is as Trustee under the terms of the Trust Agreement.
