WANITSCHKE and DORREEN M. WANITSCHKE, being first duly sworn upon <br />oath depose and state that: <br />1. Affiant Donna M. Wanitschke created the Donna M. Wanitschke Revocable Living Trust <br />upon the execution of the Donna M. Wanitschke Revocable Living Trust Agreement dated October 13, <br />2015 (the "Trust") wherein she designated herself as the initial Trustee of the Trust. <br />2. On February 1, 2019, Affiant Donna M. Wanitschke executed an Amendment to the Trust <br />wherein she amended the Trust to designate herself and Dorreen M. Wanitschke as Co -Trustees of the <br />Trust. <br />3. From February 1, 2019, Affiants have served as Co -Trustees of the Trust which Trust <br />grants Affiants as Co -Trustees all powers under the Nebraska Uniform Trust Code and further grants <br />either of the Co -Trustees acting independently or together the authority to exercise all powers granted to <br />Trustees under the Nebraska Uniform Trust Code; <br />4. The Trust is a grantor trust and may be revoked by the Affiant Donna M. Wanitschke <br />during her lifetime; <br />5. Affiant Donna M. Wanitschke is the sole beneficiary of the Trust during her lifetime and <br />Dorreen M. Wanitschke, Dee A. Hanssen, Darla S. Wanitschke and Dena M. Norman are the remainder <br />beneficiaries of the Trust upon the death of Affiant Donna M. Wanitschke. <br />
