201806415 <br />Exhibit "A" <br />Lot Three (3), Block Sixteen (16), in the Town of Wood <br />River, Hall County, Nebraska. <br />At the date of this conveyance, such real estate is owned by Janet Snodgrass, <br />a/k/a Janet E. Snodgrass. Therefore, by means of this Transfer on Death Deed, <br />Janet Snodgrass' interest in such real estate is conveyed to the Trustee of her <br />Trust, as stated, as of Janet Snodgrass' date of death. <br />At the date of this Deed, the successor Trustee of said Trust is Brenda <br />Christensen, also effective upon the death of Janet Snodgrass. Therefore, if, <br />upon the death of Janet Snodgrass, Brenda Christensen is the Trustee of said <br />Trust, then such conveyance is to her, as said Trustee. However, regardless, <br />said conveyance is to the Trustee of said Trust, as of the death of Janet <br />Snodgrass. Upon the death of Janet Snodgrass all facts regarding such Trust and <br />said Trustee may be established by an Affidavit, making reference to this Deed. <br />Warnings: <br />1. WARNING: Nebraska Inheritance Tax. The property transferred by this Revocable <br />Transfer on Death Deed remains subject to inheritance taxation in Nebraska to the <br />same extent as if owned by the Transferor at death. Failure to timely pay <br />inheritance taxes is subject to interest and penalties as provided by law. <br />2. WARNING: Medicaid. The designated beneficiary is personally liable, to the <br />extent of the value of the property transferred, to account for Medicaid <br />reimbursement to the extent necessary to discharge any such claim remaining after <br />application of the assets of the Transferor's estate. <br />The Designated Beneficiary may also be personally liable, to the extent of the <br />value of the property transferred, for claims against the estate, statutory <br />allowances to a Transferor's surviving spouse and children, and the expenses of <br />administration to the extent needed to pay such amounts by the personal <br />representative. <br />3. WARNING: Health & Human Services. The Department of Health and Human <br />Services may require revocation of this deed by a transferor, a transferor's <br />spouse, or both a transferor and the transferor's spouse, in order to qualify or <br />remain qualified for Medicaid assistance. <br />"Disinterested Witnesses".
