201806415 Exhibit "A" Lot Three (3), Block Sixteen (16), in the Town of Wood River, Hall County, Nebraska. At the date of this conveyance, such real estate is owned by Janet Snodgrass, a/k/a Janet E. Snodgrass. Therefore, by means of this Transfer on Death Deed, Janet Snodgrass' interest in such real estate is conveyed to the Trustee of her Trust, as stated, as of Janet Snodgrass' date of death. At the date of this Deed, the successor Trustee of said Trust is Brenda Christensen, also effective upon the death of Janet Snodgrass. Therefore, if, upon the death of Janet Snodgrass, Brenda Christensen is the Trustee of said Trust, then such conveyance is to her, as said Trustee. However, regardless, said conveyance is to the Trustee of said Trust, as of the death of Janet Snodgrass. Upon the death of Janet Snodgrass all facts regarding such Trust and said Trustee may be established by an Affidavit, making reference to this Deed. Warnings: 1. WARNING: Nebraska Inheritance Tax. The property transferred by this Revocable Transfer on Death Deed remains subject to inheritance taxation in Nebraska to the same extent as if owned by the Transferor at death. Failure to timely pay inheritance taxes is subject to interest and penalties as provided by law. 2. WARNING: Medicaid. The designated beneficiary is personally liable, to the extent of the value of the property transferred, to account for Medicaid reimbursement to the extent necessary to discharge any such claim remaining after application of the assets of the Transferor's estate. The Designated Beneficiary may also be personally liable, to the extent of the value of the property transferred, for claims against the estate, statutory allowances to a Transferor's surviving spouse and children, and the expenses of administration to the extent needed to pay such amounts by the personal representative. 3. WARNING: Health & Human Services. The Department of Health and Human Services may require revocation of this deed by a transferor, a transferor's spouse, or both a transferor and the transferor's spouse, in order to qualify or remain qualified for Medicaid assistance.
