201806378 Lot Fifty -Seven (57), Amick Acres West Subdivision, Hall County, Nebraska. CERTIFICATION OF TRUST PATRICIA L. JONES, being first duly sworn upon oath deposes and states that: 1. Affiant and her husband, Gerald S. Jones, created the Gerald S. Jones and Patricia L. Jones Revocable Living Trust upon the execution of the Gerald S. Jones and Patricia L. Jones Revocable Living Trust Agreement dated August 10, 2017 (the "Trust") wherein they designated themselves as the initial Co -Trustees of the Trust and further designated that upon the death of either of the Co -Trustees the surviving Co -Trustee would serve as sole Trustee and further designated Affiant's sons, Michael S. Jones, Douglas C. Jones and Bradley T. Jones, as the successor Co -Trustees of the Trust; 2. From the date of the creation of the Trust until May 1, 2018, Affiant and her husband, Gerald S. Jones, have served as Co -Trustees of the Trust which Trust grants Affiant and her husband, Gerald S. Jones, as Co -Trustees all powers under the Nebraska Uniform Trust Code and further grants either of the Co -Trustees acting independently or together the authority to exercise all powers granted to Trustees under the Nebraska Uniform Trust Code; 3. Affiant's husband, Gerald S. Jones, died on May 1, 2018 and pursuant to the Trust Affiant is now serving as sole Trustee of the Trust having the authority to exercise all powers granted to Trustees pursuant to the Trust and under the Nebraska Uniform Trust Code; 4. The Trust is a grantor trust and may be revoked by the Affiant during her lifetime; 5. Since the Trust remains a grantor trust Affiant's social security number should be used for all tax reporting purposes from and after May 1, 2018 and no separate tax identification number for the Trust has been obtained; 6. During the lifetime of Affiant, Affiant is the sole beneficiary of the Trust and Affiant's children, Michael S. Jones, Douglas C. Jones and Bradley T. Jones are the remainder beneficiaries of the Trust; 7. Affiant states that (a) there are presently and have not been since the inception of the Trust no other trustees appointed to serve on behalf of the Trust except as stated hereinabove; (b) that Affiant is the sole Trustee at this time, and (c) the Trust has not been revoked or terminated and is in full force and effect.
