His Last Will and Testament <br />dated March 19,1982, established a testamentary trust named the "Stuart W. Mieth Trust". <br />Said trust became irrevocable upon the death of Stuart W. Mieth. <br />2. Diane Henry is the currently acting Trustee of the Stuart W. Mieth Trust. <br />3. This Certificate is a true and correct representation of the terms of the trust <br />instrument. <br />4. The Will which established the Testamentary Trust is filed of record in the <br />County Court of Hall County, Nebraska, Case No. PR 84 -27, as part of the Probate of the <br />Stuart W. Mieth estate. <br />5. The Trust has not been revoked, modified, or amended in any manner which <br />would cause the representations contained herein to be incorrect. <br />6. Title to assets of the Trust should be taken in substantially the following form: <br />"Diane Henry, as Trustee of <br />The Stuart W. Mieth Trust" <br />7.
