BRUNKEN, being first duly sworn upon oath, deposes and state that: <br />1. John A. Brunken and Jeanine L. Brunken created the John A. Brunken and <br />Jeanine L. Brunken Revocable Trust by Agreement dated November 6, 2003 (the "John A. <br />Brunken and Jeanine L. Brunken Revocable Trust ") wherein they designated themselves as the <br />initial Trustees. <br />2. From the date of creation of the John A. Brunken and Jeanine L. Brunken <br />Revocable Trust until February 20, 2014, John A. Brunken and Jeanine L. Brunken served as <br />Co- Trustees of the John A. Brunken and Jeanine L. Brunken Revocable Trust. <br />3. John A. Brunken died on February 20, 2014, at which time Jeanine L. Brunken <br />became the sole Trustee of the John A. Brunken and Jeanine L. Brunken Revocable Trust. <br />4. Jeanine L. Brunken is currently serving as Trustee of the John A. Brunken and <br />Jeanine L. Brunken Revocable Trust and said Trust grants Jeanine L. Brunken as Trustee all <br />powers under the Nebraska Uniform Trust Code. <br />5. The John A. Brunken and Jeanine L. Brunken Revocable Trust is in full force and <br />effect and as of the date hereof Jeanine L. Brunken is serving as Trustee of said Trust. <br />6. The John A. Brunken and Jeanine L.
