20+006953 <br />C. The Management Powers. To maintain, repair, <br />invest, manage, insure, and in any manner deal <br />with any real or personal property, tangible <br />or intangible, or any interest therein which I <br />now own or hereafter acquire, in my name and <br />for my benefit, upon such terms and conditions <br />as my Agent shall deem proper; <br />d. Banking Powers. To make, receive and endorse <br />checks and drafts, deposit and withdraw funds, <br />acquire and redeem certificates of deposits, <br />in banks, savings and loan associations and <br />other institutions, execute or release such <br />deeds of trust or other security agreements as <br />may be necessary or proper in the exercise of <br />the rights and powers herein granted to and to <br />endorse in my name social security checks; <br />e. Motor Vehicles. To apply for a certificate of <br />title upon, and endorse and transfer title <br />thereto, for any automobile that I may own or <br />hereafter acquire; <br />f. Tax Powers. To prepare, sign and file joint <br />or separate income tax returns or declarations <br />of estimated tax for any year or years; to <br />prepare, sign and file any claim for refund of <br />any tax and to otherwise represent me before <br />any office of the Internal Revenue Service <br />(with power of substitution of any other <br />attorneys at law chosen by said Agent) for any <br />federal tax matter. <br />g. Safety Deposit Boxes. To have access at any <br />time or times to any safe deposit box rented <br />by me, wheresoever located and to remove all <br />or any part of the contents thereof. <br />2. General Grant of Power for Health Care Decisions. In my <br />Agent's sole and absolute discretion, to make all decisions that I <br />could make with respect to my health care. In exercising such <br />power, my Agent should first try to discuss with me the specifics <br />of any proposed decision regarding any medical care and treatment <br />if I am unable to communicate in any manner, however, rudimentary. <br />My Agent is further instructed that, if I am unable to give any <br />informed consent to a proposed medical treatment, my Agent shall <br />give, withhold, withdraw, or modify such consent for me based upon <br />any treatment choices that I have expressed while competent, <br />whether under this document or otherwise. If my Agent cannot <br />determine the treatment choice I would want made under the <br />circumstances, then my Agent should make such choice for me based <br />upon what my Agent believes to be in my best interests. <br />-2- <br />
