4 <br />201704501 <br />contest deficiencies determined by the Internal Revenue Service, the Nebraska Department of <br />Revenue, or other taxing authorities; to exercise any elections I may have under federal, state or local <br />tax law; and generally to represent me in all tax matters and proceedings of all kinds and for all <br />periods between 1995 and 2035 before all offices and officers of the Internal Revenue Service, <br />Nebraska Department of Revenue, and any other taxing authority; to engage, compensate and <br />discharge attorneys, accountants and other tax and financial advisors and consultants to represent <br />and/or assist me in connection with any and all tax matters involving or in any way related to me or <br />any property in which I have or may have any interest or responsibility. <br />14. To Vote Stock. To vote, in person or by proxy, all shares of common stock owned <br />by me in any corporation, at the election of directors of such corporation or on any or all other <br />matters which may require a vote of the shareholders of such corporation. <br />15. To Sign My Name. To sign my name to any document, instrument or paper, <br />including but not limited to those pertaining to my health, accident, hospitalization, disability, <br />Medicare, life insurance and Social Security benefits. <br />16. To Sell Any Motor Vehicle. To apply for Certificate of Title upon, and endorse and <br />transfer title to, any automobile, truck, pickup, van, motorcycle, or other motor vehicle, and to <br />represent in such transfer assignment that the title to said motor vehicle is free and clear of all liens <br />and encumbrances except those specifically set forth in such transfer assignment. <br />17. To Acquire Information. To receive from any person or organization any <br />7 r nation relating to my affairs or my property, and I hereby authorize and direct such person or <br />organization to release such information to my agent as if I had personally requested same. <br />18. To Open Safe Deposit Box. To have access to, to open, and to enter any and all safe <br />deposit boxes or other places of deposit, held or possessed in my name, and to deposit therein or <br />remove therefrom any or all securities, documents, or other items of personal property whatsoever, <br />whenever my agent shall deem it necessary or proper to do so; provided always that any institution <br />in which any such safe deposit box may be located shall not incur any liability to me or my estate <br />as a result of permitting my agent to exercise this power. <br />19. To Purchase Treasury Bonds. To purchase for me United States Treasury Bonds <br />which are redeemable at par in payment of federal estate taxes, and to borrow money specifically to <br />enable the purchase of such bonds. <br />20. To Access Medical and Health Insurance Records. To have access to my medical <br />records, health insurance records, and billing information related to my medical treatment; to make <br />financial arrangements for my care at any hospital, nursing home, health center, convalescent home, <br />retirement home or similar institution; and to contract for medical, hospital and nursing services. <br />Regardless of the time of commencement of the other powers granted my agent by this document, <br />I authorize all physicians and psychiatrists who have treated me, and all other providers of health <br />care, including hospitals, to release to my agent all information or photocopies of any records which <br />my agent may request. Any medical information released will serve the purpose of allowing my <br />agent to fulfill the obligations set forth in this document. All persons are authorized to treat any <br />
