Johnson and Robert Kim Johnson, being first duly sworn, depose and say: <br />1. That the Affiants are the successor cotrustees under the terms and conditions of the <br />Merleen J. Johnson Revocable Trust dated December 28, 1995 that following the death of Merleen <br />J. Johnson became known as the Merleen J. Johnson Administration Trust (hereinafter referred to <br />as the "said Trust "). <br />2. That on April 14, 2017, the Affiants became the successor cotrustees ofthe said Trust <br />following the death of Merleen J. Johnson on February 20, 2017, the settlor and initial trustee of said <br />Trust, a certified copy of her death certificate is attached as Exhibit "A ", and upon signing an <br />Acceptance to Act as Successor Cotrustees under the Merleen J. Johnson Revocable Trust, an <br />original of which is attached as Exhibit "B ". <br />3. That the Affiants remains the sole cotrustees of the said Trust as of this date. <br />4.
