<br />I further declare that Richard S. Cavenee has died and I am the sole beneficiary of the referenced
<br />Deed of Trust.
<br />I further declare that I executed a Declaration of Self Trust known as "Colleen M. Cavenee as
<br />Trustee of The Colleen Cavenee Trust (A Revocable Living Trust), a /t /a dated November 16, 2000 ".
<br />I do, by these presents, formally transfer and assign all of our right, title and interest in said
<br />Acknowledgment unto "Colleen Cavenee as Trustee of THE COLLEEN CAVENEE TRUST, u /t /a
<br />dated November 16, 2000."
