For all or any of the purposes <br />herein stated to enter into and sign, seal, execute, acknowledge, and deliver any contracts, deeds, or <br />other instruments whatsoever, and to draw, accept, make, endorse, discount, or otherwise deal with <br />any bills of exchange, checks, promissory notes, or other commercial or mercantile instruments. <br />12. To draw and endorse checks. To draw checks against my account in any bank, to <br />endorse notes, checks, drafts or bills of exchange which may require my endorsement for deposit as <br />cash or collection in any bank, to accept all drafts or bills of exchange which may be drawn upon me <br />in the usual course of my business. To receive, endorse, and cash, redeem or deposit United States <br />savings bonds and any other securities, and any checks received from the United States Treasury <br />Department made payable to me for any reason including income tax refunds, social security <br />benefits, supplemental security income, and any other benefits. <br />13. To consent to care and execute releases. To consent to any surgical operation or <br />medical treatment for me and to have me admitted to any hospital, medical facility or nursing home. <br />To execute any and all releases required by health care providers and health plans under HIPPA. <br />14. To participate in farm programs. To enter into any arrangements or contracts with <br />the United States Department of Agriculture or any of its committees to participate in any <br />agricultural programs and to sign the papers in connection therewith for me in my name and to <br />acknowledge and deliver said papers and contracts and to receive payment thereunder and to endorse <br />any checks or drafts for said payments and deposit them on my behalf. <br />15. To apply for and receive benefits. To apply for and receive for me any benefits <br />whatsoever of any federal or state government or other agency, including, but not limited to, social <br />security, medicare, veterans or welfare benefits, medicaid, etc. <br />16. To buy flower bonds. To purchase United States Treasury securities known as flower <br />bonds for me even though I may be comatose or incompetent. <br />17. To borrow and give security. To borrow any sum from any lender and encumber any <br />of my real or personal property as security for loans. To execute and deliver notes and other <br />obligations, and mortgages, security agreements, financing statement, pledges, liens, and other <br />encumbrances. <br />18. Self- dealing. My attorney may deal with my said attorney and no transaction shall <br />be void because of said self - dealing nor because my said attorney was a part owner of property with <br />me or had interests similar to mine or had any interest in said transaction. <br />19. To make gifts and implement estate plan. To make gifts to reduce my estate or for <br />charitable, religious or educational purposes. To continue any plan of gifts I have initiated. To <br />follow and implement any estate plan that I have formed. Gifts may be made to my said attorney if <br />my attorney is a member of my family and would otherwise be deprived of an equal share. <br />qe1;41 <br />
