For all other purposes, the decree becomes final and operative 30 days after the decree is <br />entered or upon the death of the parties, whichever first occurs. If the decree becomes final <br />and operative upon the death of one of the parties, the decree shall be treated as if it became <br />final and operative on the date it was entered. <br />CUSTODY, CHILD SUPPORT, VISITATION, AND TAX EXEMPTIONS <br />2. Gabriela is awarded legal and physical custody of the minor children of <br />the parties subject to reasonable rights of visitation vested in Rodolfo which parenting <br />time /visitation is set forth in a parenting plan attached hereto as Appendix 2. <br />3a. Rodolfo shall pay his child support obligation to the Nebraska Child <br />Support Payment Center, P.O. Box 82600, Lincoln, Nebraska, 68501, for the support of <br />the minor child /children in the sum of $50.00 and when there is one child the sum of <br />$50.00. See Appendix 3 attached. All payments must include the name of payor, payor's <br />social security number, county of the decree and case number. Said child support shall <br />commence on the 1st day of February, 2010, and shall continue thereafter on the first day <br />of each and every month until the respective child reaches the age of 19 years, becomes <br />emancipated, becomes self - supporting, marries, or dies, or until further order of this <br />Court. Gabriela shall claim the minor children as an exemption for tax purposes and <br />Rodolfo shall timely execute appropriate IRS forms to effectuate use of exemption. <br />4. Gabriela shall maintain health insurance for the minor child/ children. and <br />Rodolfo shall pay 0 % of any unreimbursed medical, dental, optical and orthodontia <br />expenses for the child/children in excess of $480 per year. <br />5. Rodolfo's obligation to pay child support, shall be subject to income <br />withholding. Rodolfo shall immediately prepare a Notice to Withhold Income and <br />deliver said Notice to Rodolfo's employer, if any, and provide a copy to the Clerk of <br />the District Court and to the Plaintiff. <br />6. Each party shall continue to have full and equal access to the education <br />and medical records of the minor child. <br />7. Either party may make emergency decisions affecting the health or safety <br />of the minor children while the children are in the physical custody of such parent. <br />Revised 11/25/09 <br />Page ) of <br />
