<br />1. He is the duly elected president of Grand Island
<br />Winnelson Co. and as said president, verifies this account in
<br />writing for and on behalf of Grand Island Winnelson Co.
<br />2. Grand Island Winnelson Co., by and through its duly
<br />authorized officers and agents, provided goods and materials for
<br />work done on property located at 106 Wilmar Avenue, Grand Island,
<br />Hall County, Nebraska, legally described as follows:
<br />Lots One (1) and Two (2), Martin's Subdivision
<br />in the City of Grand Island, Hall County,
<br />Nebraska.
<br />3. Goods and materials were furnished to said property at
<br />the request of Northcott Company's agent, to -wit: Shaw Plumbing,
<br />4375 N. 60th Rd., Grand Island, Nebraska 68803 -9442; between the
<br />dates of August 30, 1999 and December 14, 1999, in the total amount
<br />of Thirty Thousand Dollars ($30,000.00). Grand Island Winnelson
<br />Co. has received payment of Fifteen Thousand Five Hundred Dollars
<br />($15,500.00) towards said total amount, leaving a balance due for
<br />said goods and materials in the amount of Fourteen Thousand Five
<br />Hundred Dollars ($14,500.00).
<br />4. Grand Island Winnelson Co. claims a lien on the above -
<br />described real estate in the amount of Fourteen Thousand Five
<br />Hundred Dollars ($14,500.00), together with interest thereon at the
<br />legal rate.
