2 44307621 <br />TRANSFER ON DEATH DEED <br />evidencing a transfer effective on the grantor's death under the <br />Nebraska Uniform Real Estate Transfer on Death Deed Act <br />Neb. Rev. Stat. sections 76 -3401 through 76 -3423 <br />Jill M Strong, single, as owner, transfers on death of owner to Kay M Strong Prather <br />and Carl Prather, wife and husband, as joint tenants with the right of survivorship and <br />not as tenants in common, as grantee beneficiaries, the property at 1924 West 11th <br />Avenue, Grand Island, Nebrska, legally described as follows: <br />LOT ELEVEN (11), BLOCK FIVE (5), BOGGS AND HILL'S ADDITION <br />to the City of Grand Island, Hall County, Nebraska. <br />THIS TRANSFER ON DEATH DEED IS REVOCABLE. IT DOES NOT TRANSFER ANY OWNERSHIP <br />UNTIL THE DEATH OF THE OWNER. IT REVOKES ALL PRIOR BENEFICIARY DESIGNATIONS BY <br />THE OWNER FOR THIS INTEREST IN REAL ESTATE. <br />Pursuant to the Nebraska Uniform Real Estate Transfer on Death Deed Act, Neb. Rev. Statute sections <br />76 -3401 through 76 -3423, <br />Warnings: the Transfer on Death Deed is: <br />a. Subject to inheritance taxation <br />b. Designated beneficiary(ies) is person liable for Medicaid reimbursement and for claims <br />against transferor's estate. <br />c. HHS May require revocation to maintain qualification for Medicaid <br />d. Defect in wording of warnings does not invalidate deed <br />e.
