The Defendant shall summer visitation for fihree <br />successive weeks each summer with the summer being defined as the <br />time when school is in summer recess in the residential schoo! district <br />of �the children and said summer parenting �time shall .be contingent <br />upon the Defendant notifying the Plaintiff in writing on or.before of May <br />1 St of each year of the time he elects for his consecutive summer <br />weeks visitation. The Court further orders that on the weekend of the <br />second week of the summer parenting time, the Plaintiff shall have <br />wreekend parentir�g time with the minor childTen from Friday evening at <br />7:00 p.m. until Sunday at 5:00 p.m. <br />5. In the event that the parties cannot agree upon medical treatment, <br />education or religious education of the children, the Plaintiff shall have <br />the ultimate decision making ability. <br />6. The Defendant shall pay child support in the amount of $762.00 per <br />month beginning March 1, 2012 and on the first day of each monfih <br />thereafter until one of the minor children reaches the age of 19, enters <br />military service, dies, is emancipated or until further order of the Court <br />whichever event shall occur first. Then the Defendant shall pay <br />$654.00 per month until there is one remaining minor child. When <br />there is one remaining child, the Defendant shall pay $459.00 per <br />month child support until that minor child reaches the age of 19, enters <br />military service, dies, is emancipated or until further order of the Court. <br />Child support shall be paid through the Nebraska Child Support <br />Payment Center at P.O. Box 82600, Lincoln, NE 68501. A child <br />support calculation pursuant to the Nebraska Child Support Guidelines <br />is attached hereto and incorporated herein by reference. <br />7. In the ever�t that the Defendant fails to pay any child support as such <br />failure is certified each month by the State Disbursement Unit in cases <br />in which the Court ordered support is delinquent in the amount equal to <br />the support due and payable for a one month period of time he may be <br />required to appear in court on a day to be determined by the Court and <br />2 <br />Page_�of� <br />
