20�2001�� CERTIFICA�ION OF TRUST STATE OF MII�TNESOTA ) ) ss: COUNTY OF BENTON ) Hazold E. CampbeII and Linda S. Cooper, being first duly sworn upon oath, depose and state thax: 1. Rosemary Ca,mpbell created the Ros�mary Campbell Revocable Living Trust pursuant to Trust Agreement da.ted of June 27, 1996, as amended February 18, 2003, July 10, 2007 and May 23, 2011 (the "Trust'� wherein she designated herself as the initial Trustee and she designated Gene W. Campbell as the first successor Trustee of said Trust and designated Harold E. Campbell and Linda S. Cooper as successor CaTrustees. 2. Under date of August 29, 2006, Rosemary Campbell resigned as the initial Trustee of the Trust. Gene W. Campbell accepted. the duties of successor Trustee of said Trust by Acceptance Agreement dated August 29, 200b and from that date until June 8, 2011 Gene W. Campbell has served. as successor Trustee of the Trust. 3. Gene W. Campbell died on June 8, 2011. 4. As of the date hereof, Affiants executed an Acceptaace of Co-Trusteeship accepting the duties as Co-Tnastees of the Trust and agreeing to administer sa.id Trust in accordance with the terms of said TnYSt Agreement and tb.e laws of the Sta.te of Nebraska. 5. The Trust ie a grantor trust and as such doe�s not require a separate taxpayer identification number but rather Grantor Rosemary Campbell's personal social secu�rity number should be used in connection with all Trust matters. 6. The Trust grants Affiants as Co-Trustees a11 powers under the Nebraska Uniform Trust Code. 7. Rosemary Campbell is the sole beneficiary of the Trust during her lifetime. 8. The Trust is in full force and effect and as of the date hereof Affiants are serving as the Co-Trustees of the Trust.
