<br />q~f-/(P() <br /> <br />003.0lLl Land available for application ofpaWlch and overall <br />dimensions of the land <br /> <br />003 01 L2 Location of neighboring home sites or farmsteads and adjacent <br />land owned by others <br /> <br />0030lL3 The location of domestic water supply wells within 1,000 feet <br />of the area, the location of municipal water supply wells within 5,000 <br />feet of the area, and the approximate depths of the water tables used for <br />any such identified wells <br /> <br />003.0lL4 Location and name of watercourses or wetlands in the area <br /> <br />004 Signatures: Application and Reports. <br /> <br />004.01 All permit applications submitted to the Department and all reports required by <br />such permits to be submitted to the Department shall be signed: <br /> <br />0040lA In the case of a corporation, by a principal executive officer of at least <br />the level of vice-president; <br /> <br />004.lOB In the case of a partnership or sole proprietorship, by a general partner <br />or the sole proprietor, respectively; and <br />004 10C In the case of a municipal, state, federal or other public facility, by <br />either a principal executive officer or ranking elected official; or <br /> <br />004 lOD Bya duly authorized representative of the persons described in <br />paragraphs 004 lOA to 004 01C of this Chapter if such representative is <br />responsible for the overall operation of the activity, the authorization is made in <br />writing by the person designated in paragraphs 004 01 A to 00 01 C of this <br />Chapter, and the written authorization is submitted to the Director. <br /> <br />004 02 Any change in an authorization meeting the requirements of paragraph 004 OID <br />of this Chapter shall be submitted to the Department in writing prior to or together with <br />any application or report to be signed by an authorized representative. <br /> <br />005 Application; Additional Information Requests. <br /> <br />The thirty (30) days filing requirement for permits shall commence after all the necessary <br />information has been received by the Department. No permit application submitted to the <br />Department shall be considered for processing until all information necessary to complete the <br />application or additional information as required by the Department has been received. <br /> <br />006 Permit Conditions; General. <br /> <br />The following general conditions shall apply to all permits: <br /> <br />006 01 Duration, see paragraph QQ1 of this Chapter; <br /> <br />006 02 Retention by permittee of any records required by this Chapter for a minimum of <br />three (3) years; and <br /> <br />006 03 Responsibility of permittee to comply with all other applicable local, state, and <br />federal requirements. <br /> <br />007 Duration. <br /> <br />Each issued permit shall have a fixed term not to exceed twelve months from the date of issuance. <br /> <br />008 Transferability of Permits. <br /> <br />008 01 A permit may not be transferred. <br /> <br />QQ2. Modifying, Suspending, or Revoking Permits. <br /> <br />009.01 Any permit issued may be modified, suspended, or revoked in whole or in part <br />during its term for cause, including, but not limited to: <br /> <br />0090lA A violation of any terms or conditions of the permit. <br />
